I authorize Les-Commerces-de-la-Chaudière-Inc to make a direct withdrawal using the DPA-Desjardins system on the 15th of each billable month, or a little later in the week. Two weeks before each automated monthly payment, I will receive an email statement detailing the shipments made, the amount of the payment and confirming the date which the transaction will be made.

I prefer to pay by credit card, 3.45% charge will be applied on my bills as a transaction fee. It is my responsibility to submit by phone my credit card number, its expiry and the security at 418-623-5320 to avoid service interruption.
